Claim Assistance Guideline

Non-Medical & Motor Claim Guideline       

Immediately following such loss or damage, you should report to the Police Authorities.

When your vehicle is involved in any accident, please immediately call us, Lockton IBS Insurance Brokers (Cambodia) Co., Ltd., through our 24/7 Emergency hot-line 089 788 111 or Insurer’s Hotline for assistance and advice. Please do not make any admission, offer, promise or payment of claim without insurer’s consent.

To enable insurer to act fast, please kindly provide us the following information:

(a)               Insurance Certificate

(b)               Time, Date and Place of Loss

(c)                Preparing & providing any available proofs

(d)               Police Report

In case of theft, you are required to file a complaint to the Police immediately.

 

NOTICED OF ACCIDENT AND REQUIRED DOCUMENTS

A written notification describing the accident in full detail is important. Please submit the accident report within 30 days from the date of accident to:

1. Claim Guideline for Group Personal Accident & Hospital Insurance

 

When the Accident happened or you hospitalized or undergone surgery, you should:

A- USING INSURANCE COMPANY CLINIC PANEL NETWORK:

You may seek the treatment directly with the Insurance Companies’ Panel Network by presenting your 1-original insurance card with 2-your ID or staff card and/or Letter of Admission for Calmette Hospital. Then, the bills will be settled directly with the insurance companies. However,

 

If the medical costs exceed the limits in the schedule or your conditions of policy coverage, you will be responsible for those amounts. We will be advised by writing.

 

B- SEEKING CLAIM REIMBURSEMENT:

In case you chose to receive the treatment out of insurance companies’ panel clinics/hospital, you may need to pay out of your own pocket first and collecting the following Essential documents for claim reimbursement:

(a)  Duly completed claim forms with SIGN & STAMP

(b) Hospital or Clinic Certificate

(c)  Medical Documents (Original Invoices, X-Ray Film, Echo Results, or Blood Test Result, etc)

(d) Copy of ID Card/Passport and Copy of Insurance Card

(e) In the event of death, Death Certificate, Post Mortem Report, etc

(f)  Police Report if any

 

Upon completion, we will on behalf of you submit the documents to the insurance companies and you should receive your covered amount within the set time frame.

Motor Vehicle Claim Guideline
